  • Please complete this form, filling in the First & Last Names of all of your team’s participants as well as indicating wether each participant is a Director/Coach or a Performer.
  • Add & Remove Participants with the “+” & “-” buttons.
  • ONLY the Directors/Coaches & Performers on this list will receive ticket media, lanyards & Director/Coaches wristbands that provide each person entry into the Sports Complex for the Competition and back stage areas.
  • Once a form is submitted, you must contact your CofC Representative to make any changes.
  • Your final invoice cannot be calculated without this form, please have this form submitted by the due date.
